For Government Pharmacy Officers

This section provides centralised access to daily reporting forms, portal links, and data submission tools required of government pharmacy officers in Andhra Pradesh. Timely and accurate reporting is a core professional responsibility — it feeds into district-level health management information systems (HMIS), supports medicine procurement planning, and ensures accountability in the public drug supply chain.

Pharmacy officers posted at PHCs, CHCs, area hospitals, and district headquarters hospitals are required to submit reports at defined frequencies. Below are the key portals and forms. Bookmark this page for quick daily access.

Key Reporting Obligations at a Glance

  • Daily Stock Report: Record opening balance, receipts, issues, and closing balance for all EML medicines.
  • HMIS Monthly Report: Submit medicine consumption data to the district HMIS focal point by the 5th of each month.
  • DVDMS Portal: Update drug availability and indent status on the AP Drug and Vaccine Distribution Management System.
  • Expiry Alert Report: Flag medicines expiring within 3 months to the District Medical & Health Officer for redistribution or condemnation.
  • Stock-out Report: Immediately report any stock-out of essential medicines through the prescribed format to the APDAWCS district warehouse.
